Before You Arrive

Our office requires a referral from another physician before an appointment can be made. Before your appointment, you might need to get some labs. Please call the office in advance! Once you have an appointment, we encourage you to visit with a friend or family member. Two heads are always better than one!

Billing

Any insurance co-pays must be paid at the time of your visit. We accept check, cash or credit/debit card. Remaining balances after insurance has paid are due within 30 days. If you are uninsured or are not able to pay your balance in full, please give us a call at (541) 808-2412 so we can find a way to help!

Insurance

Be sure to bring your insurance card with you! We provide care for patients in most managed care and standard insurance plans, including Medicare. You should check with your insurance company if you have questions about whether we are on their panel of providers. We are contracted with most plans, which include:
